Print and sign the document and mail to the address on the form. If you want to let us know that you're donating (for membership purposes), you may send us a copy (simply delete your SSN for privacy).

WSU employees can use the agency code (365) that already appears on the form. If you work in Pullman, your County Code (for Whitman County) is 38. Other employees of the State of Washington can replace the agency or county code with their own code(s). Ask your CFD representative for help, or contact Bob Hoffmann at 208-883-0642.

By submitting this form, you will eliminate any previous CFD pledges you have already made. If you are already a CFD contributor, you can add RFM at http://www.cfd.wa.gov/ without eliminating your current pledges.

You may enter a one-time pledge or a recurring pledge ($2/month minimum). You can become a member by donating $4.00/month. You must notify us of your pledge if you wish to receive member benefits.
